When ordering a prescription please include your email and mobile phone number. Please complete as many fields as possible to help us. Indicate if the prescription is NEW and being emailed by your GP; NEW with an Photo of the original prescription or a REPEAT on file. Select whether you require all or some items on the prescription and if some items, please list which ones. If you have a paper prescription from your GP, please bring the original prescription with you. Thank you.
